How do I apply for the NDIS?

You can call 1800 800 110 and ask to apply for the NDIS. You can also:

  • go to the NDIS website here
  • fill out an Access Request Form here
Is TQ Care a registered provider?

Yes, TQ Care is a registered NDIS provider and successfully passed re-certification with no non-conformances.

What if I don’t like the service provider that I chose? What can I do?

You can change service providers during your plan. You will need to check your Service Agreement to find out what notice period you need to provide.

If you choose to change providers, then you will be responsible for engaging an alternative provider. Contact us today and we can let you know how we could support you. If you are funded for Support Coordination in your plan, a Support Coordinator can help you do this and they can get in touch with us.

Can TQ Care support people that have high intensity support needs?

The TQ Care team have significant experience in supporting people with complex health and disability needs. We understand the process of transitioning from hospital to home and the implementation of safe in-home supports. We work with your clinical team  members to ensure smooth, safe and quality healthcare in your home. Types of support we can provide include:

  • Medication Administration
  • Dysphasia and chronic swallowing

What is support coordination?

Sometimes finding the right support for your needs feel complicated time consuming and even overwhelming, sourcing and managing multiple providers and services is where a support coordinator can help. They can help you understand and implement your NDIS plan help you negotiate with your providers on what servers they offer and help you work towards achieving your personal goals.

What are the different kinds of support coordination?

Support Connection: this is a short-term assistance that can help participants source providers that can meet their needs and develop the responsibility to coordinate your providers and services yourself.

Coordination of supports: This is a longer-term assistance which is maintained throughout the term of the participants plan. It provides support through connecting participants to and coordinating all your supports.

Specialist support coordination: A Specialist support coordinator can support participants with more complex situations and needs, multiple stakeholders like a case manager in most cases.

Do your coordinators have experience with complex support needs?

Our support coordinators have backgrounds and experience in working in the disability sector in a wide range of roles, it’s that experience and knowledge that can help participants with complex needs feel that are in good hands as our support coordinators understand from first-hand experiences.

What’s involved in finding the right support coordinator?

Sometimes finding the right support coordinator can be overwhelming and time consuming. There are so many personal choices that need to be considered. We take our time in getting to know you and for you to get to know us, that way we can carful navigate the right fit for you and your needs.

What makes TQ Care support coordinators different from others?

We truly believe in building on your capacity to navigate the NDIS and to effectively use your plan. Its your plan and we guide you in choosing how to implement it. You are the ultimate decision maker.

How do I choose the right support coordinator for me?

Make sure ask your coordinator about their experience in the disability sector, specifically their support coordination experience. It’s a good idea to speak to the support coordinator and organise a meeting face to face or via Team or Zoom before you make your  decision. Your NDIS LAC may also assist you in choosing the right support coordinator based on your plan needs, but also based on how they communicate with you and listen to you.
